Who is Eligible?*
- Member of The Missouri Bar.
- Spouse of a Member of The Missouri Bar.
- Employee (including spouse and child, if desired) of a Member of The Missouri Bar.
Some applicants may be required to have a medical exam in order to apply for coverage.
To apply for this coverage, please complete the Group Term Life Application Form listed below and stored in Adobe Acrobat (PDF) format which can be downloaded, printed, and submitted via fax or mail.

- Spouse Coverage up to $500,000
- Employee Coverage up to $250,000
